Overcrowded wards, stressed senior physicians responsible for several wards, patients discharged prematurely to make room for new patients.
Every day I see patients being cared for in corridors behind screens. At my workplace, there are four toilets and two showers for 16 patients, and every day I see patients queuing for showers and toilets. I have seen hostels that were better equipped and prison cells that were better furnished.
Seen People Die
I have seen dysfunctional outpatient clinics. I have seen doctors crying from exhaustion and despair in the psychiatric emergency room. I have seen worn–out, experienced nurses collapse behind closed doors in a medicine storage room. I have seen how the union is being frozen out and ignored. I have seen people flee. I have seen people die. I have seen a psychiatric system that is severely affected by cutbacks, lacking stability, identity, and direction.
This has to end, we thought. Someone has to see how bad it is and do something about it! But, damn it, there were new cutbacks on the way and the department had to come up with a plan to save another few million kronor within a week. We weren't going to reopen closed wards, we were going to close new wards, we were going to lay off administrative staff.
Enthusiasts who wanted something else, who wanted more, were silenced or disappeared. "How far are you prepared to let this go?" we asked senior management. How long are you going to accept this decline? Who will protect psychiatry? Who will protect patients and staff on the floor? We were told that these were decisions from above and that it was not the role of senior management to speak out. Senior management could not answer whose role it was.
The response we received was that these issues did not even need to be discussed with us, mere doctors on the front line. We, the doctors on the front line and our immediate managers, have been trying to alert the management for many years without being heard. A tight lid was put on. A layer of insulation between decision–makers and the front line that allowed only orders and indifference to pass through. We were promised modern digital and structural solutions that would streamline healthcare, but all we have seen of these are wasted billions and a lack of dialogue, explanation, and accountability.
Amidst all this, there was one person who could loosen up the system. A true enthusiast-a senior physician, manager, and human being. A person who wanted to give everyone around them the best possible conditions.
The senior physician worked days, evenings, nights, and weekends, not because they had to, but because they wanted to. It was their calling. Diligent, constantly driven, tireless.
A Dignified Working Environment
In parallel with his role as a doctor, the senior physician held various management and advisory positions within psychiatry in Gothenburg and nationally. The senior physician was more than just a cog in the machinery of psychiatry; he was its patient architect. I started working with the senior physician in psychiatry at Sahlgrenska to see how to build a cutting–edge facility where both patients and staff feel safe. The senior physician's ultimate goal was to create the safest possible care for patients and a dignified working environment for staff and doctors in psychiatry. The senior physician loved his job, was loved by many, and was known and respected throughout the country.
However, within this system, there is no room for passionate individuals, high hopes, or bold aspirations. There is no place for outspokenness, ingenuity, initiative, or decentralization. We work within a hierarchical system governed in a distant and nonchalant manner by forces that consider themselves untouchable and above direct dialogue and accountability. Laws and regulations in the hands of these totalitarian forces become weapons used to protect themselves and attack others. If the system wanted to extinguish someone's passion, the senior physician was exactly the right person. And that is exactly what the system did. The chief physician has been suspended from his position. Taking away the chief physician's job was equivalent to depriving him of his calling in life.
Genuinely Concerned
Given the highly vulnerable position of my colleagues and immediate superiors, I would like to emphasize that I am speaking for myself when I write this. My words could be considered disloyal to my employer, and I am genuinely concerned about reprisals. However, my intention is not to harm my employer. I care deeply about psychiatry and want the very best for Sahlgrenska University Hospital.
I want to address those who are actually our employers—the Swedish people. Please help us stop this! Help us stop the dehumanization of mentally ill members of society and the demonization of psychiatry. Help us reach those who still care about more than power and money. Help us help our patients and colleagues in a way that preserves our, your, and everyone's human dignity.
The emptiness and sorrow are immense. The hopelessness is deafening. I am as guilty as everyone else, not least for my naive belief that someone will see how bad things are and save us. We are all guilty because we have allowed this monstrous system to grow.
I demand direct dialogue and accountability. I demand that psychiatry at SU be given the opportunity to heal, find its identity, and develop further on the basis of openness, knowledge, and humanism.
A great doctor and human being has left a system that currently has no place for true enthusiasts.

Ask yourself: If a criminal defendant can prove in appellate courts, beyond a reasonable doubt, that a Fair and Impartial trial was not only not given to them, but was entirely impossible from the start, due to jury tampering, jury bias, and undue influence … what happens?
Answer: The conviction and sentence, if any was obtained,
are immediately vacated, and the person is released with apologies from the government with a clear record and clean bill of health.
What else happens?
Precedent becomes established in codified stone for all other hypothetically–convicted defendants of any same/similar charges to have their own appeals granted, and upheld, all of the way up to the US Supreme Court.
Double Jeopardy then kicks in, and they can never be tried for the same crimes again.
Now think: If a defendant's name and face are plastered 24/7 across national news for weeks, with government officials publicly calling for their prosecution and engaging in reputational smear campaigns (even though such campaigns may be perfectly 'morally' justified), does that scenario meet the "burden of proof" on appeal to claim a Fair and Impartial trial was not given them, and moreover, was not possible from the start?
YES. IT DOES.
This is how you know that every talking head, both in media and public service, calling to "release all the Epstein names now!" is either grossly incompetent, or worse, actively trying to undermine the carriage–of–justice long term.
You cannot "be for the victims" of any heinous crime, and
then simultaneously participate in tainting/attempting to
taint the very constitutional Due Process necessary to put
